Chestnut Health is seeking a Nurse Practitioner to join our physiatry team serving Skilled Nursing Facilities. You will collaborate with therapists, nurses, and other clinicians to develop and implement rehabilitation plans that improve function and independence.

Formación
- Active NP license in the state of employment.
- Current DEA license and CS license required on start date.
- ANCC or AANP certification.
- Interest in Physiatry/PM&R.
- Experience in SNFs is a plus.
Responsabilidades
- Collaborate with therapists, nurses and healthcare professionals to develop rehab plans.
- Perform comprehensive patient assessments and diagnostic evaluations.
- Monitor progress and adjust treatment plans to improve function.
- Participate in multidisciplinary meetings and education activities.
- Document evaluations, plans, progress notes and discharge summaries in EMR.
- Educate patients and families about rehab process and post-discharge care.
