Qualifications

• Active licensed to practice in the state of employment.
• Current Controlled Substance License required on start date.
• Current DEA License required on start date.
• ANCC or AANP certification is preferred.
• Interest in Physiatry/PM&R and rehabilitation medicine.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other providers to develop rehabilitation plans.
• Conduct comprehensive patient assessments and evaluations.
• Monitor progress and adjust plans to optimize functional outcomes.
• Participate in multidisciplinary meetings and education activities.
• Document evaluations, plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about rehab process, outcomes, and post-discharge care.
