Chestnut Health Company is seeking a motivated Nurse Practitioner to join our physiatry team serving patients in Skilled Nursing Facilities. You will collaborate with therapists, nurses, and other professionals to create and implement comprehensive rehab plans aimed at restoring function and independence.

Formación

• Actively licensed to practice in the state of employment.
• Current DEA license required upon start date.
• Current Controlled Substance license required upon start date.
• ANCC or AANP certification as a Nurse Practitioner.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing Facility experience is a plus.

Responsabilidades

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with rehabilitation needs.
• Conduct thorough patient assessments including histories, examinations, and diagnostics to set rehabilitation goals and interventions.
• Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
• Educate patients and families about the rehabilitation process, outcomes, and post-discharge care.
