Qualifications
- Actively licensed to practice in the state of employment.
- Current DEA license required upon start date.
- Current controlled substance license required upon start date.
- Certified Nurse Practitioner certification required.
- ANCC or AANP certification.
- Desire to work in Physiatry/PM&R.
- Experience in orthopedics is a plus.
- Experience in skilled nursing facilities is a plus.
Responsibilities
- Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
- Conduct patient assessments including medical histories, exams, and diagnostics to set rehab goals.
- Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
- Participate in multidisciplinary meetings and educational activities to grow clinical skills.
- Document initial evaluations, treatment plans, progress notes, and discharge summaries in EMR.
- Educate patients and families about rehabilitation process, expectations, and post-discharge care.
