Job summary
Chestnut Health is seeking a dedicated Nurse Practitioner to join our physiatry team serving patients in Skilled Nursing Facilities. You will collaborate with therapists and healthcare professionals to design and implement rehabilitation plans, assess patients, monitor progress, and document care in the EMR.
The role emphasizes patient education, family counseling, and a commitment to improving functional outcomes and independence.
Qualifications
- Active NP license in the state of employment.
- Current DEA license required at start date.
- Current Controlled Substance license required at start date.
- ANCC or AANP certification.
- Interest in Physiatry/PM&R and rehabilitation.
- Experience in Skilled Nursing Facility setting is a plus.
Responsibilities
- Collaborate with therapists and nurses to develop and implement comprehensive rehabilitation plans.
- Conduct thorough patient assessments including histories and examinations to set rehab goals.
- Monitor patient progress and adjust treatment plans to optimize function and independence.
- Participate in multidisciplinary team meetings, case conferences, and educational activities.
- Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR.
- Provide education to patients and families about rehab process and post-discharge care.
