Chestnut Health in the United States is seeking a Nurse Practitioner to join our physiatry team serving Skilled Nursing Facilities. You will collaborate with therapists, nurses, and physicians to implement rehabilitation plans that restore function and promote independence.

The ideal candidate is licensed in Illinois, with DEA and CS licenses, and a passion for physiatry and patient-centered care. New graduates encouraged to apply and a supportive, balanced work culture is offered.

Qualifications

• Active license to practice in Illinois.
• Current Controlled Substance License.
• Current DEA License.
• ANCC/AANP Certification required.
• Interest in Physiatry/Physical Medicine and Rehabilitation.
• Orthopedic experience useful.
• Physiatry experience useful.
• Skilled Nursing experience useful.

Responsabilités

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
