Chestnut Health Company in the United States is seeking a Nurse Practitioner to join a physiatry team serving patients in Skilled Nursing Facilities. You will collaborate with therapists and nursing staff to create and implement comprehensive rehab plans, conduct assessments, and monitor progress toward independence.

Qualifications

• Actively licensed to practice in the state of employment
• Current DEA license required on start date
• Current Controlled Substance license required on start date
• ANCC or AANP certification required
• Desire to work in Physiatry/PM&R
• Experience in SNF settings is a plus

Responsabilités

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with rehabilitation needs
• Conduct thorough patient assessments including histories and exam to determine rehabilitation goals
• Monitor patient progress and adjust treatment plans to optimize outcomes and independence
• Participate in multidisciplinary team meetings and educational activities to enhance clinical knowledge
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR
• Provide education and counseling to patients and families regarding rehabilitation process and post-discharge care
