Chestnut Health is seeking a dedicated Nurse Practitioner to join our physiatry team in a Skilled Nursing Facility setting. You will work collaboratively with therapists and nurses to develop comprehensive rehab plans and assess patients for functional goals and interventions.
You will monitor patient progress, adjust plans as needed, and participate in team meetings to enhance clinical knowledge. Strong documentation and patient-family education are essential, with a commitment to work-life
Qualifikationen
- Active nursing license in the state of employment.
- Current Controlled Substance License (required upon start date).
- Current DEA License (required upon start date).
- ANCC or AANP certification is required.
Aufgaben
- Collaborate with therapists, nurses, and other professionals to create and implement rehab plans.
- Conduct patient assessments and determine rehabilitation goals and interventions.
- Monitor progress and adjust treatment plans to optimize function and independence.
- Participate in multidisciplinary meetings and educational activities.
- Maintain accurate documentation in EMR systems (evaluations, plans, progress notes, discharges).
- Educate patients and families about rehabilitation process and post-discharge care.
