Chestnut Health is seeking a Nurse Practitioner to join our physiatry team, serving patients in Skilled Nursing Facilities. You will collaborate with therapists and nurses to develop comprehensive rehabilitation plans aimed at restoring function and promoting independence.

Qualifikationen

• Active state NP license in Illinois
• Current Controlled Substance license
• Current DEA license
• ANCC or AANP certification
• Interest in Physiatry/PM&R
• SNF experience (nice to have)

Aufgaben

• Develop and implement rehab treatment plans with the rehab team
• Conduct patient assessments and determine rehab goals
• Monitor progress and adjust treatment plans
• Document evaluations, plans, progress, and discharges in EMR
• Educate patients and families about rehab process
