You will collaborate with therapists, nurses and physicians to deliver comprehensive rehabilitation care, perform patient assessments, and help develop individualized treatment plans to improve function and independence.

Qualifications

• Active NP license in Kentucky.
• DEA license on start date.
• Controlled Substance license on start date.
• ANCC or AANP certification.
• Interest in Physiatry/PM&R.
• Orthopedic or physiatry experience a plus.

Responsibilities

• Collaborate with therapists, nurses, and other professionals to develop rehabilitation plans.
• Conduct patient assessments and establish rehab goals.
• Monitor progress and adjust treatment plans to optimize outcomes.
• Participate in multidisciplinary team meetings and education sessions.
• Document initial evaluations, treatment plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about rehabilitation and post-discharge care.
