Job summary
Chestnut Health in Kentucky is seeking a Nurse Practitioner to join our Physiatry team. You will collaborate with therapists, nurses, and physicians to develop rehabilitation plans and help patients regain function.

Qualifications
- Active Nurse Practitioner license in Kentucky.
- Current Controlled Substance License.
- Current DEA License.
- ANCC or AANP certification.
- Desire to work in Physiatry/PM&R.
- Orthopedic experience is a nice-to-have.
- Physiatry experience is a plus.
Responsibilities
- Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
- Conduct patient assessments, medical histories, and examinations to determine rehab goals.
- Monitor patient progress and adjust treatment plans to optimize function and independence.
- Participate in multidisciplinary team meetings and educational activities.
- Maintain accurate documentation in electronic medical records.
- Educate patients and families about the rehab process and post-discharge care.
