Chestnut Health, a leading multi-specialty healthcare organization, is seeking a Nurse Practitioner to join our Physiatry team in Kentucky. You will work collaboratively with therapists, nurses, and other professionals to advance rehabilitation goals and patient independence.

Qualifications

• Active license to practice in the state employed.
• Current CS license required at start date.
• Current DEA license required at start date.
• ANCC or AANP certification.
• Interest in Physiatry/PM&R.
• Orthopedic experience a plus.
• Physiatry and nursing experience desirable.
• Skilled Nursing experience desirable.

Responsabilités

• Collaborate with therapists and nurses to plan rehab treatments.
• Perform patient assessments to determine rehab goals.
• Monitor progress and adjust treatment plans to optimize outcomes.
• Participate in multidisciplinary meetings and education.
• Document initial evaluations, plans, progress, and discharge summaries in EMR.
• Educate patients and families about rehab process and post-discharge care.
