Qualifications

• Bachelor's degree or higher in Physical Therapy from an accredited program.
• Licensed Physical Therapist in PA with current license.
• 1+ years healthcare experience required.
• 1+ years of experience with EMR systems (EMR, Other EMR, Practice Partner EMR).
• Valid PA driver license and ability to travel to patient homes.

Responsibilities

• Collaborate with care team to manage and deliver patient care plans.
• Provide management and delivery of home health physical therapy.
• Ensure timely charting and documentation in EMR.
• Local travel to residential homes, up to ~20% of time.
