Job summary

Residential Home Health and Hospice is seeking a compassionate Physical Therapist to manage and deliver patient care plans in Lewistown, PA and surrounding areas. You will collaborate with clinicians to optimize treatment and outcomes, with an emphasis on timely documentation and professional mobility.

The role requires a PT licensure in the state, at least one year of healthcare experience, and the ability to drive ~20% of your day. A supportive team and modern tools help support patient care.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist issued by the state of practice.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to travel ~20% of the day.
• Commitment to meet charting deadlines.
• Ability to self-manage time and scheduling.
• Experience with tablets, mobile devices, and EMR software.

Responsibilities

• Provide management and delivery of patient care plans in collaboration with the care team.
• Deliver high-quality physical therapy services to patients in Lewistown, PA and surrounding areas.
• Ensure accurate documentation and timely charting in the EMR system.

Job description

Compensation: $99,000.00 - $120,000.00, plus up to $20,000.00 in bonuses.

Hours: Monday - Friday, 8AM - 5PM, one weekend per month, two holidays per year.

We are looking for compassionate physical therapists with:

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software
