Job summary

Residential Home Health and Residential Hospice is seeking a compassionate Physical Therapist to join our team in State College, PA. The role focuses on managing and delivering patient care plans within a collaborative, team-based environment.

A valid PT license and healthcare experience are required, with about 20% travel to patient locations. The position offers competitive pay from $99,000 to $120,000 annually with bonuses up to $20,000, plus comprehensive benefits and paid time off.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college is required.
• Licensed Physical Therapist in the state where you will work.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of each day driving to/from patient locations.
• Demonstrated experience with tablets, mobile phones and EMR software.

Responsibilities

• Collaborate to manage and deliver patient care plans.
• Support patients in coordination and delivery of care in the assigned service area.
• Document charting and EMR entries accurately and timely.
• Utilize company-provided tools and mobile devices to support care delivery.

Compensation: $99,000.00 - $120,000.00, plus up to $20,000.00 in bonuses.

Hours: Monday - Friday, 8AM - 5PM, one weekend per month, two holidays per year.

We are looking for compassionate physical therapists with:

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
