Job summary

Kolmac Integrated Behavioral Health seeks a PHP Therapist to deliver individual and group therapy within the Partial Hospitalization Program, coordinating with an interdisciplinary team to support recovery.

The role includes comprehensive assessments, treatment planning, and documentation in line with DSM-5, ASAM criteria, and regulatory standards; after-hours crisis coverage may be required.

Qualifications

• Associate's degree in social work, counseling, psychology, or related field.
• Minimum 2 years of behavioral health experience, preferably PHP or outpatient.
• Experience in assessments, treatment planning, and therapy.
• Knowledge of DSM-5 diagnoses and trauma-informed care.
• Proficient in EHR and clinical documentation.

Responsibilities

• Conduct comprehensive assessments using DSM-V and ASAM criteria.
• Develop and update individualized treatment plans with the treatment team.
• Provide individual and group therapy in IOP/OP settings.
• Deliver interventions with MI, CBT, and trauma-informed care.
• Coordinate care, discharge planning, and referrals.
• Maintain timely clinical documentation and participate in meetings.
• Provide crisis intervention and participate in on-call schedule.
