Kolmac Integrated Behavioral Health is seeking a PHP Therapist to provide individual and group therapy within the Partial Hospitalization Program, coordinating with an interdisciplinary team to plan and monitor treatment. The role includes comprehensive assessments, treatment planning, and ensuring high-quality clinical documentation.

The position requires a Master’s degree in a related field and licensure (LMSW, LCSW, or LPC), with at least two years in behavioral health.

Qualifications

Master’s degree in social work, Counseling, Psychology, or a related field.
Minimum 2 years of experience in a behavioral health setting, preferably with PHP or outpatient programs.
Licenses: LMSW, LCSW, or LPC.
Experience with assessments, treatment planning, and therapeutic interventions.

Responsibilities

Conduct biopsychosocial assessments using DSM-5 criteria.
Develop and update individualized treatment plans with the interdisciplinary team.
Provide direct care through individual and group therapy sessions at PHP and OP levels.
Deliver interventions such as Motivational Interviewing, CBT, trauma‑informed care.
Coordinate care with referral sources and other providers.
Maintain accurate, timely clinical documentation in line with standards.
Participate in treatment team meetings and after‑hours on‑call coverage.
