Qualifications

• Manage daily PHP operations including staff scheduling and group sessions.
• Lead regular team meetings to review cases and coordinate care plans.
• Monitor program outcomes and ensure quality of care and patient satisfaction.
• Coordinate discharge planning and aftercare, with cross-department collaboration.
