Anthem Hospice is seeking a Hospice Care Consultant to expand referrals in the Phoenix Metro area. This role focuses on representing and promoting hospice services to increase referrals from physicians, hospitals, SNFs, and other healthcare providers.

Qualifications

• Experience driving hospice or healthcare referrals.
• Proven ability to build strong referral relationships with physicians, hospitals, SNFs and other care providers.
• Strong marketing and presentation skills with a results-driven mindset.

Responsibilities

• Functions as the primary marketing contact for referring accounts.
• Interfaces with clinicians and support staff to understand referral source needs.
• Develops a marketing plan with measurable goals and objectives.
• Conducts personal visits to referral sources on a daily basis.
• Identifies issues from referral sources and communicates them to administration.
• Sets meetings with referral sources to review agency services and results.
• Documents daily contact with referral sources.
