Job summary

Sentara CarePlex Hospital in Hampton, VA is seeking a Pharmacy Tech II-Clinical to perform clinical pharmacy care under direct supervision. Responsibilities include obtaining ED medication histories, reconciling meds, and coordinating with pharmacists, PCPs, and nursing homes.

The role supports filling meds in central pharmacy and participating in training while ensuring patient safety and regulatory compliance. Requires Virginia Tech License and CPhT, with 2+ years of experience.

Qualifications

• High school diploma or equivalent required.
• Virginia Pharmacy Tech License required.
• CPhT license (NHA or PTCB) required.
• At least 2 years of pharmacy experience required.
• Emergency Department medication history experience preferred.

Responsibilities

• Obtain medication history in the ED from patients.
• Discuss current meds with patient or caregiver.
• Call pharmacies, PCPs, and nursing homes to gather additional history.
• May work in central Pharmacy (filling meds, delivery).
• Fills and delivers medications under supervision of a licensed pharmacist.
• Triages questions outside the technician’s scope of practice.
• Ensure quality assurance and compliance with regulations.
• Use pharmacy systems to process patient information.

Overview

Location: Careplex, Hampton, VA

Full Time

Rotating Shifts: 12 hour shifts on weekdays (1200 noon - Midnight)

10 hour shifts on weekends (1100-2130)

Every other weekend

2 holidays a year (1 summer, 1 winter)

The Pharmacy Tech II- Clinical performs the technician responsibilities of clinical pharmacy care delivery under the direct supervision of a licensed pharmacist or licensed independent practitioner.

• Work in ED to get medication history from patients
• Discuss with patient or anyone with patient what meds patient currently is on
• Call pharmacies, PCP, Nursing home to gather additional med history if needed
• May also work in central Pharmacy (Filling meds, delivery of meds)
• Participates in the training of new employees
• Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner
• Ensures adequate supplies are available for continuous patient care
• Triages and escalates any questions or issues outside the pharmacy technician's scope of practice as needed
• Uses appropriate systems for processing patient information
• Ensures quality assurance is maintained according to departmental and legal requirements
• Operates pharmacy equipment appropriately
• Participates in clinical initiatives to continuously improve patient care
• Training in an advanced Clinical Pharmacy Technician job function
• In addition to the above job responsibilities, other duties may be assigned

Education

Certification/Licensure

• Virginia Pharmacy Tech License (Required)
• CPhT License (Received through the NHA or PTCB) (Required)
