Job summary

Altruix is seeking a Pharmacy Technician - Patient Care Coordinator (PCC) to support the Long-Term Care team at Crossroads Treatment Center of Carrick in Carrick, PA. This onsite, part-time role coordinates medications, supports patient care, and serves as the liaison between Crossroads, patients, providers, and the Altruix pharmacy team.

The schedule runs Tue 10:00 AM-3:30 PM, Thu 10:00 AM-2:30 PM, and Fri 10:00 AM-3:30 PM.

Qualifications

  • High school diploma or GED required with pharmacy technician registration in PA.
  • Experience in pharmacy, healthcare, or medical office settings preferred.
  • Strong communication and customer-service skills essential.
  • Familiarity with pharmacy software and systems is a plus.

Responsibilities

  • Serve as onsite liaison between Altruix and Crossroads Treatment Center of Carrick.
  • Coordinate patient medications and support pharmacy operations onsite.
  • Enter accurate patient and prescription details in pharmacy systems.
  • Assist with prior authorizations, insurance verification, and benefits checks.
  • Provide excellent customer service and coordinate care with providers and staff.

Altruix is seeking a Pharmacy Technician - Patient Care Coordinator (PCC) to join our Long-Term Care team in a part-time, onsite role supporting Crossroads Treatment Center of Carrick. You will serve as the onsite liaison between Altruix and Crossroads, working directly with patients, providers, Crossroads staff, and the Altruix pharmacy team to coordinate medications and support an exceptional patient experience.

This position reports onsite to Crossroads Treatment Center of Carrick, located at 3000 Brownsville Road, Carrick, PA 15210. This position does not require daily travel to the Altruix Uniontown pharmacy. The schedule is Tuesday 10:00 AM-3:30 PM, Thursday 10:00 AM-2:30 PM, and Friday 10:00 AM-3:30 PM.

RESPONSIBILITIES:
Onsite Liaison & Patient Support:
  • Report directly to Crossroads Treatment Center of Carrick for scheduled shifts
  • Serve as the onsite liaison between Altruix and Crossroads, helping maintain effective communication between the facility and pharmacy teams
  • Provide onsite pharmacy support and coordinate patient medication needs
  • Serve as a point of contact for patients, providers, Crossroads staff, and the Altruix pharmacy team
  • Provide exceptional customer service through daily interactions with patients, providers, and care teams
Medication Coordination & Access:
  • Coordinate specialty medication orders from intake through fulfillment
  • Enter and maintain accurate patient and prescription information in pharmacy systems
  • Support prior authorizations, insurance verification, and benefits investigations
  • Track and follow up on pending prescriptions, refills, and required documentation
  • Communicate medication needs, changes, questions, and concerns to the appropriate Altruix pharmacy team members
  • Help ensure medications are coordinated and available to patients in a timely manner
  • Collaborate with pharmacists and pharmacy technicians to support continuity of patient care
  • Support additional pharmacy and patient care coordination functions as needed
QUALIFICATIONS:
  • High school diploma or GED
  • Active Pennsylvania Pharmacy Technician registration or active Pennsylvania Pharmacy Technician Trainee registration
  • Previous experience in a pharmacy, healthcare, or medical office setting
  • Strong communication, organization, and customer service skills
  • Excellent attention to detail and ability to manage multiple priorities
  • Basic computer proficiency and willingness to learn pharmacy software
  • Ability to work independently onsite while maintaining consistent communication with the Altruix pharmacy team
  • Pharmacy Technician experience in retail, long-term care, or specialty pharmacy preferred
  • Data entry and prescription processing experience preferred
  • Prior authorization or insurance verification experience preferred
  • Experience with pharmacy management systems preferred
  • National Pharmacy Technician Certification (CPhT) preferred
