ContinuumRx is seeking a Patient Care Representative to support pharmacists and patients in Knoxville, TN. You will handle follow-ups, inventory, and refill processing while maintaining accurate records and ensuring timely deliveries.

The role requires pharmacy technician certification, strong communication, and attention to detail. Expect collaboration with clinicians, the warehouse team, and reimbursement departments in a HIPAA-compliant environment.

Qualifications

• Completion of a Pharmacy Technician training program/certification and active state license.
• Excellent verbal and written communication skills.
• Strong organizational and problem-solving abilities.

Responsabilités

• Maintain team schedule for patient follow-ups, refills, and deliveries.
• Review inventory and refill existing orders; print labels and packing lists.
• Document all progress notes promptly and accurately.
• Communicate with patients, caregivers, and clinicians; escalate concerns to the team pharmacist.
• Update patient medication profiles in EMR and present for review.
• Coordinate with Warehouse for timely supplies and deliveries.
• Notify reimbursement team of hospitalizations or status changes.
• Assist pharmacists with obtaining lab values and entering them in EMR.
• Support scheduling of equipment pickups and medical waste.
• Maintain confidentiality per HIPAA, PHI, and related regulations.
