ContinuumRx in Birmingham, AL is seeking a Patient Care Representative (PCR) to support patient communication about therapy progress and physician follow-up. The role involves updating medication profiles, obtaining inventory for refills, and assisting the pharmacist with laboratory value review.

Responsibilities include coordinating with the team for pickups, processing orders, and ensuring accurate documentation in EMR. Monday–Friday, 40 hours, on-site at the Birmingham location.

Competenze

• Pharmacy Technician training program/certification as applicable based on State regulations.
• Active license/registration as a pharmacy technician in the state of employment.
• Strong communication, verbal and written.
• Ability to work under pressure and meet timelines.

Mansioni

• Maintain team schedule for patient follow-up, refills, and deliveries.
• Process orders, print labels and packing lists for patient orders.
• Review inventory with patients to refill existing orders.
• Document in progress notes and communicate with clinicians and pharmacists.
• Coordinate with Warehouse Coordinator to ensure timely deliveries.
• Update patient medication profiles in EMR and present to pharmacist for review.
• Follow-up with physician offices as appropriate.
• Notify Reimbursement team of hospitalizations or changes in prescription status.
• Assist with lab value collection and entry into EMR.
• Support scheduling and pickup of equipment and medical waste.
• Adhere to HIPAA and protect PHI/EPHI information.
