Job summary

Penn Medicine in Philadelphia is seeking a Pharmacy Technician Care Coordinator to work under Pharmacist supervision, assisting patients with medication access and adherence, coordinating refills and delivery, and communicating with nurses, physicians, and the pharmacy team.

The ideal candidate holds a CPhT certification, PA Registered Pharmacy Technician, a high school diploma, and 1+ year of pharmacy experience; 2+ years preferred.

Qualifications

• CPhT- Certified Pharmacy Tech required.
• Registered Pharmacy Technician (PA State Board) required.
• H.S. Diploma/GED required.
• 1+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient pharmacy or payor environment (Required).
• 2+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient pharmacy or payor environment preferred.

Responsibilities

• Communicate and document appropriately and effectively with supervisors, physicians, nurse practitioners, unit based pharmacists, social workers, nurse managers, peers, patients/customers and outside agencies
• Collaboration with internal team members to gather appropriate paperwork, prescription, billing information, and/or prescription(s)
• Coordination of medication access and delivery for outpatient use
• Develop and maintain understanding of medication names and drug therapies especially for assigned service lines
• Provides appropriate follow-up relating to patients, clinical practices, and family for pharmaceutical needs and/or additional eligibility information to ensure on-going medication access
• Proficiently execute all accountabilities of Pharmacy Technician 1
