Job summary

Penn Medicine is seeking a Pharmacy Technician Care Coordinator to support medication access and adherence under Pharmacist supervision. The role involves collaborating with nurses, physicians, pharmacists, and care teams across sites to ensure timely refills and delivery for outpatient services.

The candidate will coordinate benefits, assist with copay programs, verify paperwork, and maintain drug therapy knowledge for assigned service lines, ensuring high-quality patient care.

Qualifications

  • High school diploma or GED required.
  • 1+ years pharmacy technician experience in a retail/outpatient or payor environment required.
  • 2+ years pharmacy technician experience in a retail/outpatient or payor environment preferred.

Responsibilities

  • Communicate and document appropriately with supervisors, physicians, nurse practitioners, unit based pharmacists, social workers, nurse managers, peers, patients/customers and outside agencies.
  • Collaboration with internal team members to gather appropriate paperwork, prescription, billing information, and/or prescription(s).
  • Coordination of medication access and delivery for outpatient use.
  • Develop and maintain understanding of medication names and drug therapies especially for assigned service lines.
  • Provides appropriate follow-up relating to patients, clinical practices, and family for pharmaceutical needs and/or additional eligibility information to ensure on-going medication access.
  • Proficiently execute all accountabilities of Pharmacy Technician 1.

Summary

  • Under Pharmacist supervision, the Pharmacy Technician Care Coordinator communicates with nurses, physicians, pharmacists, and other members of the medical team to assist patients with medication access and adherence. Roles may include investigation of pharmacy benefits, patient adherence evaluation, copay assistance with manufacturer programs, coordination of recurring medication refills and delivery services for select service lines. Effective communication, critical thinking, and organization skills are necessary for success in this position. The pharmacy technician communicates and coordinates with supervisors, pharmacists, peers, patients/customers, and outside agencies as necessary. The role of individuals in this position facilitates care across multiple sites and transitions in care. The successful candidate will be conscientious and focused on contributing to high quality patient care.

Credentials


  • CPhT- Certified Pharmacy Tech (Required)

  • Registered Pharmacy Tech (PA State Board of Pharmacy) (Required)


Education or Equivalent Experience


  • H.S. Diploma/GED (Required)

  • 1+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient pharmacy or payor environment (Required)

  • 2+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient pharmacy or payor environment preferred
