Job summary

AccessHealth is seeking an AmeriCorps Pharmacy Advocate to serve as a patient liaison, ensuring access to pharmacy services and addressing social determinants of health that affect medication adherence. You will engage patients at check-in, assist with program screenings, and connect them with supportive resources, both within AccessHealth and through external partners.

• High School Diploma or equivalent.
• Bilingual English/Spanish preferred.
• Strong communication and patient engagement abilities.

Responsibilities

• Educate patients about pharmacy services at check-in or in exam wait rooms.
• Assist eligibility screenings for pharmacy programs and collect documents.
• Identify SDOH factors and connect patients with services.
• Support pharmacy initiatives and health literacy through guidance on self-service tools.
• Assist with paperwork for copay coupons or assistance programs.
• Support medication synchronization programs and medication delivery support.
• Collaborate with local agencies and community leaders to address SDOH.
• Participate in QA/data reporting to improve service delivery.
• Represent AccessHealth AmeriCorps at community events; wear AmeriCorps apparel.
