Job summary

Yale New Haven Health is seeking a Pharmacy professional to ensure accurate drug distribution and timely dose delivery across the hospital. The role involves interpreting pharmacokinetic data to create monitoring plans and providing drug information to physicians, nurses, and other health care professionals.

The position requires knowledge of pharmacology, therapeutics, formulary management, and strong communication abilities to work effectively with a multidisciplinary team.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree.
• Residency in hospital pharmacy desirable but not required.

Responsibilities

• Resolve drug distribution problems ensuring accurate and timely dose delivery to all patients.
• Interpret clinical and pharmacokinetic lab data to formulate medication monitoring plans.
• Provide drug information to nurses, MDs, and other health care professionals.
• Document medication use variances (MUVs) and adverse drug reactions (ADRs).
• Educational activities within the department (written or verbal).

Responsibilities

• Resolve drug distribution problems ensuring accurate and timely dose delivery to all patients
  • Respond to telephone requests for medication in an appropriate manner, accommodating callers’ requests or routing the call to the appropriate person.
• Interpret clinical and pharmacokinetic lab data to formulate medication monitoring plans
  • Demonstrate knowledge of basic pharmacokinetic principles.
• Problem identification and resolution
  • Communicate and document problem detection, relevant findings, recommendations, and necessary follow‑up.
• Provide drug information to nurses, MDs, and other health care professionals
  • Serve as the primary source of drug information to physicians, nurses, and other health care professionals.
• Educational activities
  • Participate in and contribute to educational forums within the department (written or verbal).
• Document medication use variances (MUVs) and adverse drug reactions (ADRs)
  • Provide viable solutions to prevent recurrence of MUVs.
• Regulatory and safety quality compliance
  • Effectively demonstrate the principles and conduct consistent with a High Reliability Organization.

Qualifications

Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.

Experience

Two years of experience in an acute care hospital pharmacy is preferred for this position.

Licensure

Licensed to practice pharmacy in Connecticut (or eligible for reciprocation).

Special Skills

This individual should demonstrate knowledge of pharmacology and therapeutics, understand pharmacy systems of drug distribution, and formulary management. Excellent communication skills and the ability to work with others are required.
