Job summary

Yale New Haven Health is seeking a hospital pharmacist to join our acute care team in Connecticut. You will respond to medication requests, interpret clinical data, and help develop monitoring plans to ensure patient safety. This role requires a Pharm.D.

or B.S. in Pharmacy, CT licensure (or reciprocity), and a track record in hospital pharmacy. Residency in hospital pharmacy is desirable; two years’ acute care experience preferred.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree.
• Residency in hospital pharmacy is desirable but not required.
• Two years experience in an acute care hospital pharmacy is preferred.
• Licensed to practice pharmacy in Connecticut (or eligible for reciprocity).
• Knowledge of pharmacology, therapeutics, drug distribution systems, and formulary management. Excellent communication skills.

Responsibilities

• Resolves drug distribution problems ensuring accurate and timely dose delivery for patients.
• Interprets clinical data to formulate medication monitoring plans.
• Documents problem detection, findings, recommendations, and follow-up.
• Provides drug information to nurses, MDs, and other health care professionals.
• Participates in educational activities within the department.
• Documents medication use variances and adverse drug reactions.
• Adheres to regulatory and safety quality standards as a High Reliability Organization.

Responsibilities

• 1.Resolves drug distribution problems assuring accurate and timely dose delivery to all patients
  • 1.1Responds to telephone requests for medication in appropriate fashion accommodating callers request or routing call to appropriate person.
• 2.Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans.
  • 2.1Demonstrates knowledge of basic pharmacokinetic principles.
• 3.Problem Identification & Resolution
  • 3.1Communicates and documents problem detection, relevant findings, recommendations, and necessary follow-up.

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.

EXPERIENCE

Two years experience in an acute care hospital pharmacy is preferred for this position.

LICENSURE

Licensed to practice pharmacy in Connecticut (or eligible for reciprocation)

SPECIAL SKILLS

This individual should demonstrate knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution, and formulary management. Excellent communication skills and the ability to work with others are required.
