Job summary

Yale NewHaven Health is seeking an entry-level clinical pharmacist to support the Department of Pharmacy and patient care teams. You will review orders, ensure safe medication use, and participate in medication safety initiatives.

The ideal candidate has a PharmD or BS in Pharmacy, and CT licensure or eligibility. Hospital pharmacy experience is preferred, with a focus on CHAMP behaviors and collaboration across care teams.

Qualifications

  • Pharmacy degree from an accredited college.
  • ASHP-accredited PGY1 residency desirable but not required.
  • Connecticut pharmacist licensure or eligibility.

Responsibilities

  • Demonstrate CHAMP behaviors and promote patient safety.
  • Review medication orders considering patient characteristics, labs, and vital signs.
  • Demonstrate competency in pharmacy automation and technology.
  • Adhere to departmental policies, procedures, protocols, and guidelines.
  • Provide education and training to pharmacy staff, students, residents, and health care team members.
  • Contribute to departmental cost-reduction and revenue generation.

Responsibilities

Qualifications
Education

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree

Experience

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.

Licensure

Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut

Special Skills

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
