Job summary

Mount Carmel Health System in Columbus, Ohio, seeks a Pharmacist - Pharmacy Outpatient Care Coordinator with expertise in outpatient clinical pharmacy network development and reimbursement strategies. You will collaborate with ambulatory clinics, the Population Health Pharmacy team, and various pharmacy services to align outpatient operations with care and financial goals.

The role requires a PharmD or BSP, Ohio licensure, and 3–4 years of clinical experience, plus leadership and teaching

Qualifications

• PharmD or B.S. in Pharmacy required; advanced degrees preferred
• Active Ohio State Board of Pharmacy licensure
• 3–4 years of clinical experience
• Strong communication and leadership skills

Responsibilities

• Implement annual regional and system clinical practice and drug use improvement initiatives.
• Orient new associates to pharmacist services with training activities.
• Assess competency of patient care pharmacists and coach on high risk med programs and transitions.
• Plan and implement educational programs aligning with regional pharmacy services and strategic goals.
• Maintain documentation on practice-related educational programs per Joint Commission and OSBP guidelines.
• Engage prescribers to optimize drug use and participate in physician staff meetings.
• Contribute to hospital-based committees and medication-related work groups.
• Supervise development and performance of pharmacy interns and externs.
• Support Pharmacy and Therapeutics Committee prep including medication safety assessments.
• Assist drug procurement, including management of shortages and formulary changes.
