Job summary

Qualifications

• OHio state pharmacy licensure in good standing.
• Bachelor’s degree in Pharmacy or Doctor of Pharmacy.
• PGY2 in ambulatory care pharmacy or PGY1 with ambulatory focus.
• Board Certification in Pharmacotherapy/Ambulatory Care or 3+ years ambulatory experience.

Responsibilities

• Direct medication-related patient care and documentation.
• Ensure providers understand patient drug therapy for quality care.
• Evaluate prescriptions for legality and accuracy per laws.
• Supervise technicians and support patient service.
• Oversee medication home delivery and documentation.
• Provide patient education on medications and self-management.
• Collaborate with providers on the best treatment plans.
• Utilize EMR to support patient care and care team.
• Support PCMH as part of the care team.
• Precept students and residents; participate in training.
