Job summary

Serviam Pharmacy of Sioux City is seeking a licensed LTC Pharmacist to ensure safe medication administration and high-quality pharmaceutical care in a long-term care setting.

You will evaluate orders, maintain patient histories, and collaborate with physicians and the healthcare team to support optimal patient outcomes. A state license and PharmD or Bachelor’s in pharmacy are required, with LTC experience a plus.

Qualifications

• Bachelor’s degree in pharmacy required or PharmD preferred.
• Pharmacist licensed in the state of practice.

Responsibilities

• Evaluates physician medication orders for drug appropriateness, dosage, interactions, and administration route; makes therapy recommendations.
• Maintains patient/resident medical histories and current medications in the chart.
• Ensures appropriate laboratory monitoring for high-risk conditions and discusses results with the care team.
• Collaborates with healthcare team to provide clinical and technical support.
• Documents all physician, nurse, and patient communications in the medical record.
• Implements drug procurement, storage, distribution, and disposal policies.
• Complies with company policies, federal/state laws, and professional standards.

LTC Pharmacist

Responsibilities

• Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and facility staff as appropriate.
• Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over the counter and home remedies).
• Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with appropriate members of the healthcare team.
• Collaborates with the healthcare team to provide clinical and technical support as needed.
• Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record.
• Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs.
• Conducts job responsibilities in accordance with the standards set out in the Company’s policies and procedures, applicable federal and state laws, and applicable professional standards.

Requirements

• Education: bachelor’s degree in pharmacy, Pharm D preferred.
• Licenses/Certification: Current license in the state of practice.
• Experience in leading or working within a cycle fill or compliance-focused Pharmacy, including Long Term Care, is a plus.
• Strong people, culture-building, and leadership qualities with a clear mission orientation.
