Qualifications

  • PharmD degree and active license in the applicable state.
  • 1+ years of pharmacist experience.
  • Strong clinical judgment and attention to detail.
  • Preferred long-term care pharmacy experience.

Responsibilities

  • Perform clinical review and verification of prescriptions, evaluating interactions, allergies, dosing, renal adjustments, and duplications.
  • Conduct final medication verification for accuracy and quality.
  • Collaborate with prescribers and nursing staff to resolve medication issues and optimize therapy.
  • Provide drug information and clinical guidance to staff and healthcare providers.
  • Accept and process verbal medication orders in accordance with applicable regulations.
  • Maintain compliance with federal, state, DEA, and Board of Pharmacy regulations.
  • Oversee controlled substance management and documentation.
  • Review prescription data entry for accuracy.
  • Support workflow to ensure timely medication delivery.
  • Perform quality assurance activities and utilize pharmacy automation technology.
  • Prepare and oversee sterile and non-sterile compounded medications as needed.
  • Participate in continuous quality improvement initiatives.
  • Maintain an organized, inspection-ready pharmacy.
  • Perform other duties as assigned.

Job description

Position Summary

Reporting to the Pharmacist-in-Charge, the Staff Pharmacist responsible for ensuring the safe, accurate, and effective dispensing of medications while providing clinical oversight, medication verification, and collaborative support to healthcare providers. This role combines clinical decision-making with operational excellence in a fast-paced closed-door pharmacy serving long-term care and assisted living facilities.

Essential Responsibilities

Clinical Practice

  • Perform clinical review and verification of prescriptions, evaluating appropriateness, interactions, allergies, dosing, renal adjustments, and therapeutic duplications.
  • Conduct final medication verification to ensure accuracy and quality.
  • Collaborate with prescribers and nursing staff to resolve medication-related issues and optimizetherapy.

Physical Requirements

This role requires frequent standing, walking, bending, reaching, and lifting up to 20 pounds. Candidates must possess the visual acuity necessary for accurate medication verification, including close vision, distance vision, depth perception, color differentiation, and the ability to adjust focus. Frequent communication with healthcare professionals, staff, and customers by telephone and in person is required.

Qualifications

Required

  • Doctor of Pharmacy (PharmD)
  • Active pharmacist license in the applicable state
  • 1+ years of pharmacist experience
  • Strong clinical judgment and attention to detail

Preferred

  • Long-term care pharmacy experience
  • Experience with FrameworkLTC, PyxisMedBank, or similar pharmacy software
  • Experience with sterile or non-sterile compounding

Schedule
  • Full-time, 40 hours per week
