Qualifications

• Pharmacist II requires ACPE-accredited degree and Ohio licensure.
• Board certification in pharmacotherapy or ambulatory care within 4 years.
• Residency-trained candidates must obtain board certification within 24 months.
• ASHP preceptor qualifications must meet within 12 months.

Responsibilities

• Process prescription orders and review patient profiles.
• Prepare, dispense, and manage medications for all ages.
• Adjust therapy and order labs per consult agreement.
• Educate patients, caregivers, and healthcare staff.
