Qualifications

• Professional pharmacy degree from an ACPE-accredited College of Pharmacy.
• Unrestricted license to practice in the State of Ohio or eligible for licensure.
• Board Certification in Pharmacotherapy or Ambulatory Care Pharmacy or equivalent advanced certification.
• Residency trained candidates must obtain board certification within 24 months.
• Meet ASHP preceptor qualifications within 12 months.

Responsibilities

• Process prescription orders; review patient profiles; prepare and dispense medications.
• Manage medications to support safe, effective drug therapy.
• Adjust therapy and order labs per consult agreement when appropriate.
• Provide education to patients, caregivers, and healthcare staff.
