Froedtert Health in Milwaukee is seeking a Pharmacist to promote safe, effective medication use on-site at Froedtert Hospital. The role requires independent decision making and collaboration with the healthcare team.

Minimum 1 year hospital experience or 3 years other pharmacy experience; Bachelor’s or Pharm.D.; Wisconsin license or eligible within 6 months.

Shift 3, overnight, 0.875 FTE, 35 hours; benefits and career opportunities available.

Qualifications

• Pharmacist license (WI) or eligibility to obtain within 6 months.
• Bachelor’s degree in Pharmacy or Pharm.D. from ACPE-accredited program.
• ASHP or pharmacy society involvement preferred.

Responsibilities

• Promote safe and effective medication use and consider therapy costs/benefits.
• Collaborate with the healthcare team to address medication therapy issues.
• Ensure patient care compliance with Joint Commission/Medicare/Medicaid standards.

Job Summary:

Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.

EXPERIENCE DESCRIPTION:

A minimum of 1 year hospital or 3 years of other pharmacy experience is preferred.

EDUCATION DESCRIPTION:

Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.

SPECIAL SKILLS DESCRIPTION:

Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.

LICENSURE DESCRIPTION:

Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
