Job summary

Froedtert & MCW Health Network in Menomonee Falls, WI, is seeking a Pharmacist for a hybrid, full-time role at our Integrated Service Center. You will apply independent decision making to promote safe, effective medication use and collaborate with the healthcare team to address therapy issues.

The position requires a Wisconsin Pharmacist license (or eligibility within 6 months), 1 year hospital experience or 3 years in other settings, and ACPE-certified degree.

Qualifications

• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy.
• Active Wisconsin Pharmacist License required or eligible to obtain within 6 months.
• 1 year hospital experience or 3 years of other pharmacy experience preferred.
• Demonstrates effective use of MS Word, Excel, Outlook; PSW/ASHP membership preferred.

Responsibilities

• Promote safe and effective medication use and consider therapy costs and benefits.
• Interact with healthcare team and patients to address medication therapy issues.
• Ensure patient care aligns with quality improvement plans and Joint Commission/Center for Medicare/Medicaid standards.

Shift: 1

Shift Details: 8 hour shift Monday through Friday between the hours of 6:30am and 6:30pm, no weekends or holidays

Job Summary

Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.

Experience

A minimum of 1 year hospital or 3 years of other pharmacy experience is preferred.

Education

Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.

Special Skills

Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.

License

Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
