Job summary

Froedtert Health in Milwaukee, WI is seeking a Hospital Pharmacist to promote safe, effective medication use and collaborate with the health care team. You will address medication therapy issues and ensure care adherence to quality standards.

Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months. On-site, 40 hours weekly with standard shifts, and opportunities in professional development and clinical collaboration.

Qualifications

• Must have Pharmacy degree and eligible for Wisconsin licensure.
• ASHP residency or PGY1 preferred.
• Strong computer application skills (Word/Excel/Outlook) relevant to practice.
• Professional membership in PSW/ASHP or similar organizations preferred.

Responsibilities

• Promote safe and effective medication use and assess therapy value.
• Collaborate with the health care team to address medication therapy issues.
• Ensure care quality aligns with Joint Commission/Medicare standards.
• Maintain documentation and communicate effectively with patients and staff.

Shift Details: 8 hour shifts, 2 holidays per year, 1 weekend every 4 weeks

Job Summary:

Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.

EXPERIENCE DESCRIPTION:

A minimum of 1 year hospital or 3 years of other pharmacy experience is preferred.

EDUCATION DESCRIPTION:

Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.

SPECIAL SKILLS DESCRIPTION:

Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.

LICENSURE DESCRIPTION:

Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
