Trinity Health through Saint Francis Hospital in Wilmington, DE seeks a Pharmacist in the Emergency Room to provide clinical pharmacy services, medication histories, and emergency response within the hospital.

The role requires hospital pharmacy experience, a Pharm.D. degree, Delaware licensure, and certifications (BLS/ACLS). The position offers a full-time schedule with rotating shifts and weekend rotations, plus comprehensive benefits.

Qualifications

• Pharm.D. from ACPE-approved college required.
• ASHP-accredited PGY-1 or 3 years hospital experience required.
• Registered pharmacist in Delaware required.
• Preferred: Pyxis MedStation, EPIC experience.
• BLS and ACLS certification before orientation (provided if not available at hire)

Responsibilities

• Assesses patient data to identify medication problems via profile reviews, pharmacy consults and rounding.
• Participates in medical emergencies and alerts such as trauma, stroke, sepsis and STEMI as required.
• Reviews provider orders to ensure drug, dosage form, and dose are appropriate for age/weight/comorbidities; updates medication history.
• Consults with providers and nurses on pharmacotherapy optimization to improve patient outcomes.
• Provides drug information and policy guidance to staff and patients for safe medication use.
• Documents patient care activities in the EMR per department guidelines.

Shift: 40hrs/wk. 11am-7:30pm Monday-Friday (with weekend rotation)

Location: Saint Francis Hospital - Wilmington, DE

Position Purpose

This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.

As a Pharmacist, you will:

• Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
• Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
• Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. Updates medication history for all admitted patients.
• As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
• Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
• Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record as per departmental guidelines.

Minimum Qualifications:

• Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
• Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
• Registered pharmacist in Delaware required.
• Preferred: Experience with Pyxis MedStation, EPIC
• BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)
