Job summary

Lumicera Health Services in Madison, WI is seeking a Pharmacist, Clinical Operations to join our team. This role conducts telephonic patient consultations, performs medication reconciliation, and ensures accurate order verification prior to shipment.

You will interact with prescribers and patients to verify information and expedite processing. The ideal candidate has ACPE accreditation, an active license, and 2+ years of pharmacist experience.

Qualifications

• Graduate of ACPE-accredited School of Pharmacy with a PharmD or B.S. Pharmacy degree.
• Current, unrestricted Pharmacist license in the state where on-site work is conducted.
• At least 2 years of licensed pharmacist experience preferred.

Responsibilities

• Verify prescriptions for specialty medications and maintain detailed records.
• Provide telephonic patient consultations and side-effect management guidance.
• Perform medication reconciliation and ensure accurate order verification prior to shipment.
• Interact with prescribers and patients to verify prescription information and expedite processing.
• Maintain compliance with quality standards and department production metrics.
