Job summary

Trinity Health Mid-Atlantic seeks a licensed CNA to provide personal care for elderly participants in a Broad Street facility, with occasional home care visits. Responsibilities include supporting daily living activities, mobility, and basic health reminders.

You will work in a patient-centered environment, with opportunities for development, using advanced technology and participating in an Employee Referral program.

Qualifications

• Active certified nurse assistant (CNA) licensed in the relevant state.
• Minimum 1 year of documented experience with frail or elderly population preferred.
• Valid driver’s license.
• Superior written/verbal communication skills.
• Customer-focused with ability to adapt to change.
• Proficiency with Microsoft Office and basic email/calendaring.

Responsibilities

• Provide personal care assistance for activities of daily living.
• Assist participants with physical activity, exercise, and walking.
• Provide behavioral supervision and support, reminders for nursing/medication, diet restrictions, and emotional reassurance.
• Assist with conversations, recreation, meals, snacks, and housekeeping.

Employment Type: Full time Shift: Description: Personal Care worker Shift: First Shift Status: Full time Location: Broad Street

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. This position will be housed primarily at the Broad Street facility. However, occasional home care work may be required.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred
• Valid driver's license
• Superior written/verbal communication skills
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems
• Position may require 10% local travel and use of personal vehicle.
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.
• Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
