Trinity Health PACE in the United States is seeking a Nurse Practitioner to join our PACE team. You will perform assessments with PO physicians, build care plans, and ensure comprehensive care for participants while maintaining a warm, supportive environment.
You will also participate in discharge planning, treat acute illnesses, conduct home visits, contribute to clinical policies, and share on-call responsibilities within the center’s geographic region.
