Trinity Health Mid-Atlantic is seeking a part-time Per Diem Personal Care Assistant/Home Health Aide to support LIFE St Mary in Feasterville-Trevose, PA. The role focuses on assisting elderly participants with ADLs, mobility, medication reminders, and companionship, with occasional home care duties.

The position emphasizes independence, flexible scheduling (4p-8p), and commitment to compassionate, person-centered care within a coordinated care model.

Qualifications

• Must be an active CNA in the state where PACE Organization is located.
• Minimum of one year of experience with frail or elderly population preferred.
• Valid driver's license.
• Superior written/verbal communication skills.
• Customer focused, able to adapt to change.
• Proficiency with MS Office and basic email/calendar familiarity.
• Position may require 10% local travel and ability to lift 20 pounds.

Responsibilities

• Provide personal care assistance maintaining independence in ADLs (toileting, showers, eating, grooming).
• Assist with physical activity, exercise, and walking.
• Provide behavioral supervision or support and reminders for nursing/medication instructions, diet, and emotional reassurance.
• Assist with meals, snacks, recreation, and light housekeeping.

Employment Type: Part time Shift: Description: The personal care assistant/worker is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. Home Health Aide Shift: 4p-8p Status: Per Diem Location: LIFE St Mary, 2500 Northgate Dr, Feasterville-Trevose, PA Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. This position will be housed primarily at the LIFE St Mary facility. However, occasional home care work may be required.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred
• Valid driver's license
• Superior written/verbal communication skills
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems
• Position may require 10% local travel and use of personal vehicle
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting
• Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable
