Job summary

Trinity Health Mid-Atlantic is seeking a CNA to provide hands-on personal care for elderly participants at the St Joseph PACE facility in Mishawaka, IN. The role emphasizes independence in activities of daily living, mobility support, and compassionate patient-centered care.

Requirements include an active CNA license in the applicable state, at least one year of geriatric care experience, and a valid license.

Qualifications

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience with a frail or elderly population preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personal care assistance to maintain participant independence in activities of daily living.
• Assist with mobility, exercise, and walking for participants.
• Offer behavior supervision and reminders for nursing and medication instructions, diet restrictions, and emotional support.
• Support with daily activities including meals, recreation, housekeeping, and social interaction.

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. This position qualifies for a $2500 sign on bonus with a 2-year commitment. The 3 installments are paid out over the two-year commitment period. This position will be housed primarily at the St Joesph PACE facility.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred.
• Valid driver's license.
• Superior written/verbal communication skills.
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change.
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems.
• Position may require 10% local travel and use of personal vehicle.
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.
• Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
