Join to apply for the PERSONAL CARE WORKER role at Upham's Community Care.

Assist a designated participant of UESP to live at home safely by providing home management, personal care, exercise, social support, and escort services, as indicated by the participant and the identified needs in the participant's individual care plan.

Duties & Responsibilities

• Assist with Activities of Daily Living (ADL): bathing, grooming, dressing, undressing, feeding, toileting, walking, transferring in and out of bed, chair, or tub.
• Assist with medications.
• Support exercise therapy including walking and range of motion; use and care of equipment such as Hoyer lift, wheelchair, commode, tub seat.
• Perform home management tasks: cooking, shopping, laundry, housekeeping, money management.
• Provide social support: escort to appointments, recreation activities.
• Complete required paperwork.
• Communicate with supervisor and staff.
• Attend in-services as determined by supervisor.

Minimum Basic Knowledge

• Ability to follow care plan.
• Good rapport with participants.
• Ability to communicate in the participant's language.

Experience & Qualifications

• Experience working with elders in their homes.
• Ability to provide services as per care plan.

Independent Action

• Provide care at home.
• Report observations to clinical supervisor.
