Job summary

Hospice Care in Wooster, Ohio, is seeking a dedicated Personal Care Specialist (CNA) to support patients in their homes. The role emphasizes comfort, dignity, and patient-centered care with collaboration from nurses and the hospice team.

Full-time position with a Monday–Friday schedule, 8:00 a.m.–4:30 p.m., plus weekend and holiday rotation. Local travel within Wooster service area required and driving is essential.

Qualifications

• High school diploma or GED required.
• Current Ohio STNA/CNA certification preferred.
• At least one year of healthcare or direct patient-care experience preferred.
• Basic computer skills and ability to use electronic documentation systems.
• Valid driver's license and safe driving record.
• Reliable transportation and state-required automobile insurance.
• Ability to travel locally between patient homes.
• Hospice certification (CHPNA) encouraged within two years of employment.

Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
• Provide compassionate, respectful, and patient-focused care.
• Observe patients for changes in condition and promptly report concerns to the appropriate member of the care team.
• Accurately document care and services provided using electronic systems.
• Promote patient safety, comfort, privacy, and dignity.
• Communicate effectively with nurses, caregivers, family members, and other members of the hospice team.
• Develop trusting relationships with patients and their loved ones.
• Follow the individualized plan of care and hospice policies and procedures.

What You'll Do
