Job Overview:

The PACE Personal Care Assistant (PCA) is a member of the PACE Interdisciplinary Team (IDT). The PCA is responsible for providing personal care services in accordance with individual care plans for PACE participants. The PCA also performs other tasks as assigned by PACE Center management.

Job Requirements:

• High School Diploma or GED (Preferred)
• Up to 1 year experience working with a frail or elderly population (Preferred)
• Must have reliable transportation

Job Responsibilities:

• Provide personal care services as per participant care plans, including peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.
• Assist with food service delivery and monitoring during meals at the PACE Center.
• Implement interventions as directed by IDT disciplines, including mechanical assistance for transfers.
• Accompany participants to outside medical appointments as assigned.
• Monitor participants for health or psychosocial changes and communicate these promptly to the IDT or management.
• Attend IDT and morning meetings as scheduled.
• Complete PCA documentation in the PACE EHR according to regulations and organizational policies.
• Support and participate in the PACE QI program and activities.

Working Conditions:

• Frequent bending, kneeling, stooping, and lifting over 50 lbs.
• Consistent concentration, communication, and use of hands.
• Occasional climbing, reaching, color vision, and visual acuity.
• Frequent sitting, standing, walking, and pushing/pulling.
