Job Overview:

The PACE Personal Care Assistant (PCA) is a member of the PACE Interdisciplinary Team (IDT). The PCA provides personal care services according to individual care plans for PACE participants and performs other tasks as assigned by PACE Center management.

Job Requirements:

• High School Diploma or GED (Preferred)
• Up to 1 year experience working with a frail or elderly population (Preferred)
• Reliable transportation

Job Responsibilities:

• Provides personal care services such as peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation, following care plans.
• Assists with food service delivery and monitoring during meals.
• Implements interventions as directed by IDT disciplines, including mechanical assistance for transfers.
• Accompanies participants to outside medical appointments as assigned.
• Monitors participants for health or psychosocial changes and reports promptly.
• Attends IDT and morning meetings as scheduled.
• Completes documentation in the PACE EHR per regulations and organizational policies.
• Supports and participates in PACE QI activities as assigned.
