Altra Homecare of Kokomo is seeking a Personal Care Assistant to deliver in-home care in Rensselaer, IN. You will assist with daily activities, monitor health, provide reminders for medications, and support patient nutrition and mobility under supervision.

We value compassionate individuals with at least 1 year of experience, CPR certification, and reliable transportation. The role offers a flexible schedule and competitive pay in a community-based homecare setting.

Qualifications

• Must be 18 years or older and able to read, follow written instructions, and document care provided.
• CPR certification is required.
• Preferred: minimum 1 year of relevant work experience.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in patient condition to supervisor.
• Prepare light meals and assist with feeding.
• Monitor and record patient temperature.
• Assist patients with ambulation.
• Conduct ROM and simple physical activities.
• Maintain documentation and adhere to care protocols.
• Follow privacy and infection control guidelines.
• Offer bathing assistance.
• Perform light housekeeping.
• Provide companion services, including letter-writing and reading.
