Altra Homecare Group is seeking compassionate Personal Care Assistants to join our team in Wabash, IN. You will deliver essential personal care services tailored to each patient’s needs in a home setting.

Responsibilities include medication reminders, safety support, light meals, documentation, and companionship, with a focus on patient comfort and dignity. A valid driver’s license and CPR are required, with benefits provided.

Qualifications

• Must be 18 years or older.
• Possess a valid Driver's License and Auto Insurance.
• Current CPR certification.
• Preferred: 1 year of relevant work experience.
• Ability to read, follow written instructions, and document care.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in the patient's condition to supervisor.
• Prepare light meals and assist with feeding.
• Monitor and record patient temperature.
• Assist patients with ambulation.
• Conduct ROM exercises and simple physical activities.
• Maintain documentation and adhere to care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance.
• Perform light housekeeping.
• Provide companion services including letter-writing and reading.
